This application is designed to support high school students with disabilities who have an Individualized Education Program (IEP). The IEP outlines the specific special education services a student receives. By completing a brief 10-minute survey, users receive a customized draft of their Postsecondary Transition Plan (PTP), which is a key component of the IEP. The PTP serves as a strategic outline to facilitate the student’s transition into adult life after high school. Users have the option to email their draft transition plan to themselves or others for review and further development.

The PTP is essential for guiding students as they prepare for life beyond high school. It aims to assist students in achieving their personal goals, whether that involves employment, training, continued education, or independent living. Effective transition planning ensures that students receive appropriate services and supports tailored to their aspirations and needs.

This app functions as a resource for Wisconsin students, families, and educators during the transition planning process. It aligns with the requirements set forth by the Individuals with Disabilities Education Act (IDEA) - http://tinyurl.com/jcvoaz3. For Wisconsin students aged 14 or older, or family members supporting such students, this tool provides guidance for preparing for IEP meetings where transition plans, including the PTP, are discussed and formulated. While the app is freely accessible to users outside Wisconsin, note that transition plan requirements may vary by state.

The app was originally developed by the Disability Rights Center of Kansas, a nonprofit organization. It has been adapted by the Transition Improvement Grant (TIG) staff in Wisconsin to meet local transition planning needs.
